Electronic participation is mandatory for companies with more than 10 workers, but given the ease of filling and automatic sending to Caravela, we suggest that it is always used.

Whenever an accident victim goes to a Caravela health insurance network, it must be accompanied by the accident claim - provider version - that it can issue after submitting an electronic claim.

After submitting the electronic submission, it is immediately forwarded to Caravela. The policyholder may then print a copy of the complete claim and another copy with only the relevant information to deliver to the clinical provider.

If the number of employees of the company allows the participation on paper to be filled, use the CONTACT FORM to send the report to Caravela.

DOCUMENTATION

Whenever the victim has received the first assistance outside of the Caravela's agreed network, when he goes to it, he must be accompanied by all the clinical documentation and exams he has in his possession.

HOLIDAY LEAVES

When reporting the claim, make sure that Caravel has vacation leaves for the month of the accident and at least the previous 6 months.

This way, we will be able to fulfil our obligations regarding the payment of disability compensation.
